Dear parents,

Please return the signed Immunization consent forms as soon as possible.

If you did not receive one in the mail, please come in and sign one before December 19,2008.

If you take your child to the doctor to get the Tetanus ,Diptheria and Pertussis Tdap vaccine , please remember to send me a copy of the record by May 5,2009.

Help me help you complete this requirement.
